Care Home Support Service

This service is for all residents within Hillingdon Care Homes for older adults.
Matrons, GPs and Advanced Nurse Practitioners visit the care homes regularly to ensure that residents are keeping well and provide medical support if needed. The team assess and treat residents in the comfort of their home wherever possible and ensure that people have an appropriate care plan which has been developed with the patient, their carers and families.
Matrons call the care homes each morning and will discuss any residents who may need to be seen that day. If necessary, the matron will arrange an appointment - this could be by telephone with a GP or as a visit to the home. Practices can also book appointments for their patients.
What are the hours of operation?
The service runs 9am to 6pm Monday to Friday.
Are all appointments carried out in person as a home visit?
The Doctor or Advanced Nurse Practitioner will speak with the patient first to see if a home visit is required.
