Managing your High Blood Pressure

What is Hypertension?

High blood pressure, also called hypertension. This is when the pressure in your blood vessels is unusually high. If the pressure is consistently too high, it puts a strain on your heart and could lead to strokes, heart attacks and kidney damage. Things that increase your chances of high blood pressure include being overweight, having an unhealthy diet, smoking, and not exercising enough. 

What is Hypertension Plus?

Hypertension Plus is a service to help patients with high blood pressure, monitor themselves and get support when they need it.  The service uses a medical app called Viso, which allows patients to monitor their blood pressure at home. The app will soon be available to all patients across Hillingdon.

If you are using the app and you have consistently high blood pressure readings, you will be asked to contact your GP.  If you have very high BP readings, Viso automatically directs patients to seek urgent care. It is free for patients to use and requires a smartphone.

Who is this for?

Patients who are 40 years and over

And have one or more of the below:

  • Have a regular blood pressure measurement over 140/90 mmHG
  • Have previously had a stroke or TIA (Transient Ischaemic Attack)
  • Do not already have a diagnosis of AF (Atrial Fibrillation)
  • Raised BMI of 30 plus and/or chronic kidney disease (ckd stage 3) and or cardiovascular disease (CVD)

Do I need a smart phone or tablet?

Yes, any android or apple phone or tablet released after 2013 will be compatible.

How many times a week do I need to submit readings?

To be able to produce an accurate average, you will need to submit a minimum of 4 readings.
