What is a PCN?
PCNs (Primary Care Networks) are groups of GP practices working closely together - along with other healthcare staff and organisations - providing integrated services to the local population.
In Hillingdon we have 6 PCNs

What are neighbourhoods?
Neighbourhoods provide integrated care - joining up services from primary and secondary care, community services, social care, voluntary sector and local groups.
Within neighbourhoods we develop services based on need which improves the quality of patient lives. They support empowering patients to understand how to manage their health and wellbeing to avoid escalating care needs.
We recognise that collaborative working with our partners, shared resource allocation, contract management and joint risk management, best supports PCNs to deliver its ambition to reduce health inequalities and improve health and care outcomes.
